Community Health Workers (CHWs)

Community Health Workers (CHWs) have expertise in helping individuals connect to local services within the community. CHW services include assistance with applying for OHP health benefits, connecting with mental health and substance use treatment, housing applications, health assessments, with medical and dental services, and much more. The goal of the CHW program is to assist members of the community in improving their health and wellness.
