Intellectual Developmental Care Coordination (IDD)
Services Coordination
Service Coordinators (Case Managers) assist with assessing support needs, designing and implementing person-centered support plans, and authorizing services for eligible adults and youth to meet those needs. Case Managers will monitor service providers to ensure quality care is being provided in a healthy and safe environment and provide direct referrals to assist clients who may need a residential placement or foster home.
Adult Protective Services
An APS Investigator will respond to all allegations of abuse and neglect of all persons eligible for services through the Community Developmental Disabilities Program. Protective services provided during a screening or investigation can include client advocacy, consultation, family support, and crisis intervention.
Family Support
Family support services are provided to assist our clients and their families in obtaining the services and support necessary to achieve growth and maintain independent living.
